of the guy wires were not attached to anchors. This exposed employees to
hazards and injuries
received should the rig tip over.
One method of abatement would be to assure that all guy wires are attached
and if anchors could
not be located near the site, that either the owner of the lease be
contacted and an anchor(s) be
installed or portable anchors could be carried on the rig and installed
when and where they are
required.
