Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: M D Self #1 Lease, Pecos Valley Oil Field, near Imperial, Texas - Well Servicing Rig Texas License #16P 373: On or about 26 March, 1996. The employer did not provide an auxiliary method of escape from the tubing boards of the derrick for employees working at that location. This exposed employees to fire and fall related hazards and injuries resulting from fire and/or explosions in the well. One method of abatement would be to assure that an alternate escape method is installed per industry standards immediately after erection of the mast.ion

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: M D Self #1 Lease, Pecos Valley Oil Field, near Imperial, Texas: On or about 26 March, 1996, the employer failed to lock-out electrical control panel which provided power to the well being worked on exposing employees to struck-by hazards (being struck by the counter-weights should the power be turned on) and related injuries. One feasible and accepted method would be the use of a lock that only one person on the crew has access to, and that person would be responsible for locking the power in the off position at the start of the job and assuring that all equipment and personnel are clear of the equipment prior to start up.rt

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The %% employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: M D Self #1 Lease, Pecos Valley Oil Field, near Imperial, Texas - Well Servicing Rig Texas License #16P 373: On or about 26 March, 1996, the employer did not adequately secure the rig from tipping over by using all the guy wires required and provided with the equipment, four of the guy wires were not attached to anchors. This exposed employees to hazards and injuries received should the rig tip over. One method of abatement would be to assure that all guy wires are attached and if anchors could not be located near the site, that either the owner of the lease be contacted and an anchor(s) be installed or portable anchors could be carried on the rig and installed when and where they are required.
