Section 5(a)(1) of the Occupational Safety and Health Act of 1970:: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: Panhandle Concrete Company Inc; 2001 W Amarillo Blvd; Amarillo, Texas: On or about 8 May, 1996, a front end loader (Catepillar Model 930, s/n 41K6679) was in use at the employer's facility moving sand and gravel from a bulk storage area on the east side of the employer's property to the conveyors loading the scale/hopper. This was at a distance of approximately 200 feet, without an (a) operational reverse signal alarm, and (b) a fire extinguisher. These conditions exposed employees to (a) being struck by the front end loader if standing behind the unit, the engine hood obscuring the view behind, and (b) burns, should the engine/fuel system ignite during fueling or electrical problems. Among other methods, one feasible and acceptable method of abatement to correct this hazard is (a) install and assure operational capability of the back-up alarm, and (b) install, using permanent brackets, an industrial type fire extinguisher in or near the cab of the equipment.er
