Section 5(a)(1) of the Act:  Each employer shall furnish to each of his
employees
employment and a place of employment which are free from recognized
hazards that are
causing or likely to cause death or serious physical harm to his
employees:
On May 5, 1994 an employee, while operating a self-propelled elevating
work platform was
exposed to hazardous chemicals such as, but not limited to, 2,4 toluene
diisocyanate, and
approximately 150 psi pressure when the platform he was operating
burst/fractured a pipe
The employee had not been trained on the operation, limitation of and/or
performance of a
self-propelled elevating work platform rented from Big "R" Rental company.
Feasible and useful abatement methods for reducing this hazard include,
but are not limited
to:
1.  Complete training for an employee who is expected to operate a
self-propelled
elevating
work platform.
2.  Use a two man buddy system to aid in the navigation of the platform
while it is operation
