Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
(a)  The hazard of being struck or squeezed by an elephant on
or about December 15, 1992, while removing the elephant leg chains.
One feasible and acceptable abatement method, among others, to
correct these hazard(s) would be to enforce and establish the
following procedures:
1)  Enforce policy requiring daily entries in the zookeeper
daily report when there is a change in temperament or behavior of
elephant.
2)  Enforce policy requiring written evaluation by the
Elephant Management Committee when incidents occur because of change
in termperament or behavior of elephant.
3)  Establish written procedures for evaluating performance
of elephant handlers.
