On or about June 8, 1995, a gate fell from it's track and rollers causing
multiple injuries
to an employees shoulder and rib cage as the gate was being opened.  The
gate was
located in the south end of the pallet storage area and had fell off
several times before
and placed back on the track by Maintenance personnel.  Employees were
exposed to the
hazards of being struck by a falling gate if it fell from it's track and
rollers.
