employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in that employees were expoed to:

planking and the outside edges of form work during a concrete
pour thus exposing employees to falls of 8 feet onto
reinforcing steel andform ties below their work level.
Among others one feasible and useful means of abatement would be to
provide
scaffolds around the entire perimeter of the form and provide wider
platforms as walkways.
