The employer did not furnish employment and a place of employment
which were free from recognized hazards that were causing or likely
to cause death or serious physical harm to employees in that
employees were exposed to:
The core mixer hoist is supported by a wooden beam which has a
substantial crack in it.  This crack may compromise the integrity
of the hoisting mechanism.  One feasible method of correction would
be to have the hoist support looked at by a professional engineer
to determine the load capacity and adjust accordingly.
