Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)The hazards of a crushing injury due to being caught between the pin
lifter
handle and the end of the car body during coupling and uncoupling
operations.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to ensure that the pin lifter handle
is installed with a minimum 2 inch clearance from the end frame
of the car body as specified by 231.1(k)(2)(v) of Railroad Safe
Appliance Standards as referenced by 49 CFR 231.18 of that same
part.

(a)The hazards of falling from moving railroad equipment during
operations with the 25 ton Ohio Locomotive Crane, S/N 4546,
Model E.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to ensure that all sill steps
(stirrups) are properly aligned and maintained as specified in 231.1
et. seq. Railroad Safety Appliance Standard as referenced by 49
CFR 231.18.
