Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
(a)the hazards of being crushed by portions of the trash compactor
mechanism of hydraulically actuated front end loading trash compactor
trucks, due to uninformed drivers' entry into the truck body or placing of
portions of their body/bodies within the hazard zone while power was on
to the unit.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to maintain a formal program that
provides for the periodic monitoring of the methods employed by
front end loader drivers for entering front loading trash compactor
trucks for cleaning purposes, and provide additional training and/or
discipline as needed to correct any inappropriate behavior detected,
as specified by paragraph 6.1(b) of American National Standards
Institute standard Z245.1-1992 American National Standard for
Refuse collection, processing and disposal equipment-Mobile
Refuse Collection and Compaction Equipment-Safety
Requirements.

591009, due to an unintended activation of the mechanism by water or
other foreign materials in the housing of the "Compact" and/or "Retract"
limit switches during maintenance and/or cleaning operations.
Among other methods, one feasible and acceptable abatement
method to correct this hazard is to properly maintain all electrical
control circuitry wiring in proper condition, as specified by
paragraph 6.1(d) of American National Standards Institute standard
Z245.1-1992 American National Standard for Refuse collection,
processing and disposal equipment-Mobile Refuse Collection and
Compaction Equipment-Safety Requirements.
