Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a) The Manitowoc crawler crane, model #4600, s/n 460061, which had not
been tested and
certificated by an accredited person or his authorized representative.
Crane
was used to hoist
leg sections of an offshore jackup drilling rig to be connected to leg
already in place.
Employees were exposed to the hazard of being struck by falling portions
of the load, or
projectiles caused by falling load, or failure of crane.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is
to maintain written, dated and signed inspection reports and records of
annual examination of
cranes in accordance with Paragraph 5-2.1.3 of the USA Standard,
B30.5-1968, crawler,
locomotive and truck cranes.
