ORS654.010:  The employer did not furnish a place of employment which was
safe and healthful
for employees, and did not do every other thing reasonably necessary to
protect the life, safety,
and health of the employees:
(a)  Oregon Department of Transportation.
The employees and especially the victim were not provided with
specific/special precautions to
take while working at the job site located in the center median of I-5 at
Milepost 283.  They
were not informed of the access/egress to the location, through a
pedestrian walkway that had
been provided, (they chose to run across the freeway lanes).  There was
not designated parking
areas, so some employees parked on the freeway off/on ramps.
Other issues include, but not limited to:  on-site safety inspections are
not being carried out with
contractors, enforcing PPE requirements, and training employees in the
avoidance of hazards
associated with the construction site(s).
