Title 40.1-51.1.(a), Code of Virginia: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that: (a) In the sheet metal department, Crown Simplimatic, Inc. failed to ensure that the foot control pedal of the Verson press brake, SN 24549, was guarded against accidental contact. "One feasible and acceptable abatement method, among others, to correct this hazard, according to ANSI B.11.3.4.3.4.3, is to cover the top and sides of the pedal."4.3.4.3,
