40.1-51.1.(a):  The employer did not furnish employment and a place
of employment which were free from recognized hazards that were
causing or likely to cause death or serious physical harm to
employees in that employees were exposed to:
The Virginian Golf Club, Jonesboro Road, Bristol, VA -  The park
brakes on the John Deere "Turtle"; Model #1800; S/N M01800G010210,
and the Kawasaki "Mule" Model # KAF620C; S/N KIAFCC18RB500489, were
not in working condition as prescribed in ANSI B71.4-1990; Safety
Specifications for Commercial Turf Care Equipment; Section
6.4.3.1.1.
One feasible and acceptable abatement method, among others, to
correct this hazard is for the employer to purchase and install the
parts required to make the park brakes functional.

employees in that employees were exposed to:
The Virginian Golf Club, Jonesboro Road, Bristol, VA -  The
employer allowed their employees to operate a Ransomes "Turf Trac"
Tractor; Model 420; S/N G3130, with blower attachment while the
steering wheel was broken and held together with duct tape in three
different areas.  The broken steering wheel was unstable and could
limit the operator's ability to safely drive the tractor.
One feasible and acceptable method, among others to correct this
hazard is for the employer to follow the manufacturer's
recommendations contained in the operator's manual and replace the
worn and damaged steering wheel with a new one.

causing or likely to cause death or serious physical harm to
employees in that employees were exposed to:
The Virginian Golf Club, Jonesboro Road, Bristol, VA -  The
employer allowed their employees to operate a John Deere; Model
1145; S/N M01145X010268, mowing tractor (equipped with a rollover
protection structure) with a seat belt that was not adequate for
restraining the operator in the event that the tractor overturned
in that the defective seat belt could be pulled a part using
minimal force.
One feasible and acceptable method, among others to correct this
hazard is for the employer to remove the existing seat belt system
and replace it with a new seat belt system that will adequately
restrain the operator.

employees in that employees were exposed to:
The Virginian Golf Club, Jonesboro Road, Bristol, VA -  The
employer allowed their employees to operate a John Deere Utility
Vehicle (Turf Care Equipment); Model 1800; S/N M01800G010210, while
the speed control lever was restrained in the fast position with a
piece of wire.  ANSI Standard B71.4-1990; Section 6.1.3.6 states
that speed control mechanisms must have a neutral position from
which the control is actuated.
One feasible and acceptable abatement method, among others, to
correct this hazard is for the employer to have the speed control
lever repaired.
