A.  Employer knew or should have known that employees operating the 2Y
operating
unit in
the manual mode, on or about January 26, 1999, were exposed to the hazard
of being burned
by heated caustic soda mixture.  A feasible and useful means of abatement,
among others, is
to install on automatic steam shut-off device to shut the steam off when
operating the 2Y unit
in the manual mode.l
