Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees; and Specifically, on December 11, 1998, the CCI crew did not: A. Have a plan to avoid damage to the underground gas line in and near the construction area, B. Maintain a clearance between the underground gas line and the cutting edge of the anchor being installed, C. Uncover or support the underground gas utility line while they performed an anchor installation, and D. Install the anchor in a careful and prudent manner.
