Minn. Stat. 182.653 subd. 2: The employer did not furnish to each employee, conditions of employment and a place of employment free from recognized hazards which caused or were likely to cause death or serious injury to employees: The work platform used on the forks of a forklift to elevate employees, which was available for use at the main building storage area, did not have a means to secure the platform to the forklift mast on the right side and the left side had a chain that was not securely attached in a manner that would prevent it from being torn loose from the platform. The platform could be displaced from the forks allowing it to fall to the ground with the employee aboard.e
