Minn. Stat. 182.653 subd. 2:  The employer did not furnish to each
employee, conditions of employment and a place of employment free
from recognized hazards which caused or were likely to cause death
or serious injury to employees; Reference ANSI B11.6-1984(R1994),
Sec. 7.1.1 Instruction of employees.  The employer shall train
and instruct the setup personnel, setup operator, operator,
and helpers in the safe method of performing any operation on
the type of lathe the employee will be required to operate or
setup.
Sec. 7.1.2 Supervision.  The employer shall ensure by
supervision that safe operating procedures are being followed.
Sec. 7.1.4.  Personal Protective Equipment. The employer shall
determine what personal protective equipment is needed when
operating lathes and shall enforce the proper use of this
equipment.
On April 22, 1998 the operator was wearing gloves while
operating the 14" Leblond lathe #03-01673.
Abatement guidelines:
1) Develop a written policy on the use of gloves while
operating lathes.
2) Conduct personnel training on the proper operating
procedures as outlined in the operator's manual.
3) Document this training with who conducted it, when it was
conducted, who attended and a brief summary or outline of what
was covered.
4) Develop another method of smoothing rotating shafts besides
using hand held emery cloth.
