Title 40.1-51.1(a) Code of Virginia:  The Burruss Company failed to
furnish to each employee a place of employment which was free of
recognized hazards that are causing or likely to cause death or
serious physical harm to employees, in that employees were exposed
to potential flying wooden objects and potential struck by hazards
where:
(a)  At this establishment, the Irvington Moore Transfer Car
(Serial No. P307-13-G-1060) was not used in accordance with the
manufacturers recommendations.  A 4" X 4" wood post was used in
lieu of the steel bar that was supplied with the transfer car.
Instance #1:  The transfer car on the east side of the kilns was
using a 4" X 4" post to push the lumber into the kiln in lieu of
the steel push bar as recommended by the manufacturer.
Instance #2:  The transfer car on the west side of the kilns was
using a 4" X 4" post to push the lumber into the kiln in lieu of
the steel push bar as recommended by the manufacturer.

One feasible and acceptable method of abatement, among others, to
correct this hazard is install a push bar that rides in the track
on the deck of the transfer car, as provided by the manufacturer.

exposed to potential tipover hazards and crush injuries:
(a) At this establishment, seatbelts, installed on the Hyster 50
powered industrial truck, were not worn by employees when operating
the powered industrial trucks.

One feasible and acceptable method of abatement, among others, to
correct this hazard is to require forktruck drivers to wear
seatbelts while operating powered industrial trucks, and enforcing
this requirement.t,

furnish to each of his employees safe employment and a place of
employment which was free of recognized hazards that were causing
or likely to cause death or serious physical harm to his employees,
in that employees were exposed to potential lacerations and crush
injuries where:
a) "Bird Caged" and kinked wire rope was being used to pull up to
ten thousand (10,000) pounds of lumber from the transfer car into
the kilns located at the west side of the kiln area.  This wire
rope was also being used to pull the stacks of lumber off of the
yard onto the transfer car.

One feasible and acceptable abatement method would be develop a
detailed inspection procedure of the wire rope to prevent this
condition from recurring.
