40.1-51.1.(a):  CCL Container (Harrisonburg) Inc did not furnish
employment and a place of employment which was free from recognized
hazards that were causing or likely to cause death or serious
physical harm to employees in that employees were exposed to
crushing injury or amputation due to the incidental activation of
the jog button during set procedures on the capper line #7.
On September 3, 1998, an employee workin on the capper line, cycled
the machine by inadvertently activating the start button.
This hazard is recognized throughout the industry; by ANSI and
1910.212 Machine Guarding. Two examples of guarding are jog
switches are two-hand tripping devices, and key activated jog
switches.
"One feasible and acceptable abatement method, among others, to
correct this hazard is providing a hinged flip cover over the jog
button that must be lifted to depress the button preventing the
unintentional start-up during Set-up procedures."
