40.1-51.1(a) of the Code of Virginia:  Storeman Specialty, Inc. did
not furnish employment and a place of employment which were free
from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were
operating a Chicago Hydraulic Press Brake, Model #1012-L; Serial
#L-17961; 60-90 Tons, and a Niagara Shear, which are located in the
metal shop, which exposed them to:
(a)  Amputations by not having a disconnect switch capable of
isolating the press brake and control system from the main power
supply.  The press brake has an on and off switch but it is not
capable of being physically locked out.  Among other methods, one
feasible and acceptable abatement method to correct this hazard is
to install a disconnect switch to the press brake such as that
called for by ANSI B11.3-1982, American National Standard Safety
Code for Power Press Brakes.
(b)  Amputations by not having a magnetic starter to prevent the
press brake from automatically restarting upon restoration of
power.  Among other methods, one feasible and acceptable abatement
method to correct this hazard is to install a drive motor so that
the operator would have to restart the press brake upon restoration
of power such as that called for by ANSI B11.3-1982, American
National Standard Safety Code for Power Press Brakes.
(c)  Amputations by not performing daily, weekly, monthly and
annual inspections and maintaining inspection records to ensure
that all of their parts, auxiliary equipment, and safeguards are in
safe operating condition on the press brake.  Among other methods,
one feasible and acceptable abatement method to correct this hazard
is to implement a daily, weekly, monthly, and annual inspection
list for what should be inspected for the above described time
frames and, to maintain these inspection records such as that
called for by ANSI B11.3-1982, American National Standard Safety
Code for Power Press Brakes.
(d)  Amputations by not providing the operator training on the
press brake to provide knowledge of the function and purpose of
safeguarding and the dangers of unsafe work practices, inattention,
horseplay, and misuse of equipment.  Among other methods, one
feasible and acceptable abatement method to correct this hazard
would be to implement operator training on the power press brake
such as that called for by ANSI B11.3-1982, American National
Standard Safety Code for Power Press Brakes.
(e)  Amputation by not providing a cover for the foot treadle on
the press brake and the shear to prevent accidental actuation by
falling or moving objects, or by someone stepping on it.  Among
other methods, one feasible and acceptable abatement method to
correct this hazard would be to provide a cover over the foot
treadles for the press brake and the shear such as that called for
by ANSI B11.3-1982, American National Standard Safety Code for
Power Press Brakes and ANSI B11.4-1983, American National StandardSafety
Code for Shears.
(f)  Contusions and abrasions by not providing adequate
safeguarding to the back of the shear to prevent the operator and
other employees from injury by being too close to the stock as it
completes its cycle.  Among other methods, one feasible and
acceptable abatement method to correct this hazard would be to
provide an awareness barrier on the back of the shear to prevent
employees from coming too close to the machine such as that called
for by ANSI B11.4-1983, American National Standard Safety Code for
Shears.
