General-duty citation text
40.1-51.1(a) of the Code of Virginia: McQuay International, did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were operating two Cincinnati shears, #702 and #703, and, four Cincinnati Hydraulic Press Brakes, #698; #717; #716, and #715, which were located in area P12, which exposed them to: Amputations by not providing adequate safeguarding to the back of the shears and press brakes to prevent the operator and other employees from injury by being too close to the point of operation. Among other methods, one feasible and acceptable abatement method to correct this hazard would be to provide an awareness barrier on the back of the shears and press brakes to prevent employees from coming too close to the machine such as that called for by ANSI B11.4-1983, American National Standard Safety Code for Shears and Press Brakes.
