Section 31-370(a) of the Connecticut Occupational Safety and Health Act:
The employer did
not furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazards of atmospheres that were immediately
dangerous to life
or health during interior structural fire fighting operations without
provisions for emergency
standby personnel.

(1)Fire fighter were allowed to begin interior fire attack on an interior
structural fire, that was beyond the incipient stage, and the required
minimum of two "outside" (standby) persons were not available to provide
emergency assistance as needed, to those in the hazard area.
Among other methods, one feasible and acceptable abatement methokd
would be to:
a)Only allow members who arrive on scene of a working structural
fire prior to the assembling of four persons, to engage in exterior
actions such as establishment of a water supply, the shutting off of
utilities, the placement of ladders, the laying of attack line to the
entrance of the structure or exposure protection.
