Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:

El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
At the main passageway:  Where employees transit, a leak was observed in
the
roof that
pass by a fluorescent lamp, by where the water flows through the lamp
creating a short
circuit and then falling in the floor.
Hazard:  Slips, falls and contact with electrical energy.
En el pasillo principal:  Por donde transitan los empleados, se observo
una filtracion en
el techo que pasa por una lampara fluorescente, por donde el agua pasa por
la lampara
creando un corto circuito y luego cae en el piso.
Riesgo: Resbalones, caidas y contacto con energia electrica.,

At the Program of Medical Assistance Office:  Five instances of leaks in
the roof were
observed, from which the water fell to the floor.
a)  In the main passageway, by where the employees transit, a leak in the
roof
was observed.
b)  In the certifiers office, were they work, a leak in the roof was
observed.
c)  In the dates by mail office, where the dates are send to the clients,
a leak in
the roof was observed.
d)  In the screening area, where the employees makes the screening, a leak
in the
roof was observed.
e)  In the archive area, where the employees put into archives the records
of the
clients, a leak in the roof was observed.
Hazard:  Slips and falls.
En la Oficina del Programa de Asistencia Medica:  Se observaron cinco
casos de
filtraciones en el techo, desde las cuales caia agua al piso.
a)  En el pasillo principal, por donde los empleados transitan, se observo
una
filtracion en el techo.
b)  En la oficina de los certificadores, donde estos trabajan, se observo
una
filtracion en el techo.
c)  En la oficina de citas por correo, donde se le envian las citas a los
clientes,
se observo una filtracion en el techo.
d)  En el area de pre-seleccion, donde los empleados que hacen la
pre-seleccion
trabajan, se observo una filtracion en el techo.
e)  En el area de archivo, donde los empleados archivan los expedientes de
los
clientes, se observo una filtracion en el techo.Riesgo:  Resbalones y
