Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:

El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
At the cutting area: There was an employee cutting bank vouchers, the
guillotine trade
mark and serial number (Polar Eltromat #60498) was design to be operated
with two
buttons was changed to be operated by the employee with just one button.
Hazard: cuts and/or lost of an extremity
Among other methods, one feasible and acceptable abatement method to
correct this
hazard is to operate the guillotine with the two buttons that where
originally designed and
to eliminate any alteration of the original design.
En el area de corte: La guillotina de marca y numero de serie (Polar
Eltromat #60498)
disenada para operarse con dos botones fue alterada para ser operada por
el empleado
por un solo boton  el cual se encontraba cortando formas bancarias.
Riesgo: cortaduras y/o perdida de extremidades
Entre otros metodos, un metodo factible y aceptable para corregir este
riesgo es el de
operar la guillotina por los dos botones en que fue originalmente disenada
y eliminar
cualquier alteracion al diseno original.
