At the mixed concrete manufacturing Plant Number # 1: On august 9, 1998 the
employer's safety manager send an instruction report to every mix
manufacturing plant
stating that all vehicles shall be equiped with an audible warning device
or reverse signal
alarm audible above the surrounding site noise level; on august 21, 1998,
an employee
was impacted by truck number 3 69, which to this date was not equiped with
a reverse
signal alarm. The affected employee was carried out to the state
Compensation Fund
Corporation Hospital where he was diagnosed head, forehead and face
trauma, nose
abration, left shoulder sprain, right and left forearm trauma and
lumbosacral sprain, all
injuries impact related according to the attending physician. Up to the
inspection date on
september 14,1998, seventeen (17) trucks, including number # 69, remained
without been
equiped with reverse signal alarms.
among other methods, one feasible and acceptable abatement method to
correct this
hazard is to inspect all vehicles for reverse  signal alarms and install
this equipment on
those vehicles detected with fault.
En la Planta de concreto mezclado Numero # 1: En 9 de agosto de 1998, el
gerente de
seguridad del patrono envio un informe instruccional a cada planta de
manufactura de
mezcla estableciendo que todos los vehiculos tenian que estar equipados
con un aparato
de precaucion audible o alarma de retroceso audible sobre el nivel de
ruido del sitio; el
21 de agosto de 1998, un empleado fue pactado por camion numero # 69, el
cual hasta
esa fecha no estaba equipado con una senal de alarma de retroceso. El
empleado afectado
fue llevado al Hospital de la Corporacion del Fondo del Seguro del Estado
donde le fue
diagnosticado trauma de la cabeza, frente y la cara, abrasion en la nariz,
torcedura del
hombro izquierdo, traumas en los antebrazos derecho e izquierdo y
torcedura lumbo-
sacral; todas esta lesiones relacionadas con el impacto, arrastre y caida
al suelo, de
acuerdo con el medico que le atendio. Hasta la fecha de inspeccion en 14
de septiembre
de 1998, diecisiete (17) camiones, incluyendo el numero # 69, permanecian
sin haber
sido equipados con senales de alarma de retroceso.
Entre otros metodos, un metodo accesible y aceptable para corregir este
riesgo es
inspeccionar todos los vehiculos para senales de alarma de retroceso e
instalarlas en
