Section 6(a) of the Puerto Rico Occupational Safety and Health Act:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or physical harm to employees in
that employees were
exposed to:

El patrono no
proveyo empleo y un sitio de empleo libre de riesgos reconocidos que esten
causando o que
puedan causar muerte o dano fisico a los empleados, por cuanto empleados
estaban expuestos
a:
At the Scorpio Recycling Inc. facilities in the feeding and compactation
area, employee
was operating the Lefort compacting machine, Model 27-6246 and serial No.
28-96-006.
The machine was reported with defect in the ram actuation joystick. But
remained in use
with the defect.  Among other methods to correct this hazard is to repair
the
machine
before it is used again.
Hazard: Malfunction of the machine.
En las facilidades de Scorpio Recycling Inc., en las facilidades de
alimentacion y
compactacion, empleado estaba operando la maquina compactadora Lefort,
Modelo 27-
6246 y No. de serie 28-96-006. La maquina fue reportada con defecto en la
palanca de
actuacion del piston, pero continuo operandose con el defecto.  Entre
otros metodos para
corregir este riesgo es reparar la maquina antes de que esta sea usada
nuevamente.
Riesgo:  Mal funcionamiento de la maquina.
