A.R.S. 23-403.A.: The employer did not furnish to each of his employees employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that : PVC pipe was running on the ground and was used to transport compressed air. American National Standards Institute/American Society of Mechanical Engineers (ANSI/ASME) standards B31.8-1986, Gas transmission and distribution piping Systems, limits the operating pressure of plastic piping Distribution Systems to 100 p.s.i. and prohibits the installation of such systems above ground except where the above ground portion of the plastic service line is completely enclosed in a conduit or casing of sufficient strength to provide protection from external damage and deterioration. Also the Clary Component Saw air brake system was disconnected due to the cost of wear on the motors in which the saw would not stop immediately per the manufacturers specifications. (a) Truss plant area: Approximately 40' of 1/2" schedule 60 PVC was being used to supply compressed air at 100 p.s.i. to various air line fittings in the truss area. (b) Component saw room, Clary Component Saw: The emergency air brakes on the saw were disconnected allowing the 15", 16" and 21" blades to continue rotating after power was shut off.
