General-duty citation text
T.C.A. 50-3-105(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees. An example on 1/15/2000 was that a Second Shift Supervisor and a Cuber Operator were exposed to carbon monoxide (from about 4:30 PM to about 10:15 PM) from the block plant kilns (fed by the exhaust gas and steam from the gas-fired Cure Pak Steam Generator in a room on the side of the plant) to the extent that these employees experienced symptoms of dizziness, nausea, vomiting, and unconsciousness. Such symptoms related to blood carboxyhemoglobin levels (30and 40) that caused and were likely to cause serious physical harm. (Note: coma reportedly can result from carboxyhemoglobin concentrations of 50, and death reportedly can result from carboxyhemoglobin concentrations of 60) Also, on 1/17/2000, first shift employees in the block plant were exposed to carbon monoxide concentrations that were likely to cause death or serious physical harm when the gas-fired Cure Pak Steam Generator was again run at the block plant. Murfreesboro Fire Department personnel reportedly measured carbon monoxide concentrations ranging from 200 ppm to 900 ppm in the block plant on that day. One feasible and acceptable abatement method to correct this hazard is to strictly adhere to the "Cure-Pak Operating Specifications" and "Cure-Pak Maintenance Schedule" in the "Cure-Pak Steam Generator Installation and operation Instructions," which were originally supplied by the company on 4/18/90 and which have been updated with the "CurePak Operation Specifications" and "Maintenance Schedule," which were sent to you by the Service Manager of Johnson Gas Appliance Company on 2/12/00. Monitoring of carbon monoxide levels in the Block Plant, with alarms for excessive carbon monoxide concentrations, will alert employees of the need for evacuation. NOTE: this hazard was corrected during the course of the inspection, as verified by carbon monoxide monitoring conducted on 2/7/2000 and 2/18/2000.
