General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) On March 11, 1988, at the Windmill Lakes Project, wooden roof trusses being erected at various units were not adequately braced to prevent a truss collapse; exposing employees to the hazards of being struck by, or falling from collapsing trusses. The following deficiencies were noted: 1) There were no diagonal braces anywhere on the top chord. 2) The first trusses were not braced sufficiently to prevent dominoing. A single brace on one side at the center was all that was utilized. 3) No knee braces were used at the end or first trusses, for needed extra support. 4) The top chord lateral braces were short 3/4 x 2 x 30 inch plywood spanning only (2) two trusses each. Minimum 2 x 4's, as long as can be handled are required. 5) The lateral braces were fastened to the top chord with a single 8 p nail at each end. Double leaded 16p (2) two each brace end are required. 6) No truss web bracing was utilized. 7) No bottom chord lateral bracing was used. 8) Poor storage of trusses which may allow them to tip or topple. Among other methods, one feasible and acceptable method of abating these hazards is to install and store trusses in accordance with the requirements of the Truss Plate Institute, contained in B.W.T-76, Bracing Wood Trusses.
