Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
a) Employees were exposed to the hazards of confined space entry
including oxygen deficiencies, toxic atmospheres, and possible fire
or exposions while repairing or other work within the furnace, where
employees are required to enter.
A feasible method to correct this hazard among other methods is to
implement a confined space entry procedure which as a minimum consists
of the following:
1) Implement a written confined space entry precedure.
2) Require testing of atmospheres for explosive nature and oxygen
content.
3) Provide adequate ventilation to reduce lack of oxygen or flammable
vapors.
4) Provide adequate training of employees including emergency procedures
rescue and use of safety equipment.
5) Provide standby safety observers to communicate with workers.
6) Provide adequate lockout of system.
7) Keep adequate records of training safety drills, atmospheric testing
and inspection of equipment available for entry or emergencies.
