free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:

from the valve as it is being opened by reaching over or around the
valve wheel is located at arms length from the edge of the kettle.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is to replace the valve and install it with the
valve wheel turned outward.
