General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Falls of 20 feet or greater to the ground from fixed ladders located throughout the facility on conveyors C1, C2 and C3 areas. Ladders were at least 20 feet in length and were attached by only two bolts at the top and two bolts at the bottom leaving the middle section unsecured, thus allowing the potential for excessive stress on the failure of the attachment bolts. One feasible and acceptable means of abatement would be to assure that the ladders have a maximum of 10 feet spacing of supports for fastening the side rails to the structure.
