Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Main Bay, Furnace Area; employees are exposed to furnace explosions and molten metal runouts in that the safety features (leak detectors, low water pressure, over temperature devices) on the4000#, 600 KW Tri Line Inductoherm Corp. electric furnace are inoperable. One feasible method of abatement, among others, is the repair and development of a preventative maintenance program in accordance with the furnace manufacturer's instruction manual to insure the correct and proper functioning of the furnace's safety features.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Pattern Storage/Squeezer Area; employees are exposed to being struck by a falling hoist and/or a molten iron ladle in that the runway for the Yale 1 ton overhead hoist used in pouring operations is suspended from the building structure with 5/8 inch chains. One method of abatement, among others, is the thorough examination of the track and supports by a qualified engineer and the installation of suspension fitting and supporting structure in accordance with AISC Manual of Steel Construction.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Grinding/Cleaning Area; employees were exposed to being struck by a falling load due to hoist failure in that a Yale 1 ton hoist is used to lift pots, from U.S.S. Fairless Works, weighing 2315 pounds. No preventative maintenace inspection program is conducted on this hoist One method of abatement, among others, is the implementation if a frequent and periodic inspection program on the hoist and the elimination of hoist over-loading.
