(b) On or about 11/8/94, Service Area
Mechanics used a screwdriver to engage and disengage the carriage safety
lock on
Benwil Industries Inc Automotive Lift Serial #B9005016.  The handle of the
carriage safety lock for the lift was completely broken off.
ABATEMENT NOTE:  Among other methods, one feasible method to reduce this
hazard is to establish and enforce adequate Maintenance and care
procedures such as
those stipulated in "American National Standard for Automotive Lifts:
Safety
Requirements for the Construction, Care and Use" (ANSI B153.1-1990).
Elements
essential to an acceptable safe maintenance and care procedure include at
a minimum:
1.Inspect  automotive lifts prior to use to ensure that safety locking
devices
and all other components are in safe operating condition.  Any lift found
to be defective or in need of repair should be taken out of service
immediately and the malfunctioning, damaged, broken or worn parts
should be repaired or replaced.
2.Train operators concerning all the appropriate procedures for the safe
operation and use of this type of machinery.
3.Post manufacturers operating manuals in the service area for information
and reference.
