Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by equipment operating in reverse having an obstructed view to the rear: a)NSP Area, Caterpillar, front end loader No. 966B, Co. No. 502N: machine was operated without a reverse signal alarm audible above the surrounding noise level or an observer signaling it was safe to operate machine in reverse, exposing employees to traumatic injuries, October 13, 1994. b)NSP Area, Retech Screen Areas, Caterpillar Front End Loader No. 980B, Co. No. 511E: machine was operating without a reverse signal alarm audible above the surrounding noise level or an observer signaling it was safe to operate machine in reverse, exposing employees to traumatic injuries, October 13, 1994. c)Picker Area, two Caterpillar Dump Trucks D250: trucks were operated without a reverse signal alarm audible above the surrounding noise lever or an observer signaling it was safe to operate trucks in reverse, exposing employees to traumatic injuries, October 13, 1994. AMONG OTHER METHODS, SOME FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE: 1.Equip and maintain a reverse signal alarm audible above the surrounding noise level on equipment. OR 2.The equipment is backed up only when rear observer signals that it is safe to do so.
