Section 5 (a) (1) of the Occupational Safety and Health Act

of 1970:  The employer did not furnish employment and a
place of employment which were free from recognized hazards
that were causing or likely to cause death or serious
physical harm to employees in that employees were exposed
to: traumatic injuries when operating the Welchang Elevator
Works freight elevator that was not provided with a door or
gate with electric contacts at the entrance to the car. The
elevator was located at the rear area of the Warehouse and
operated between the first and second floors. (State
elevator inspection certificate was expired 10/30/90). Among
other methods, one feasible and acceptable abatement method
to correct this hazard is to install a door or gate at the
entrance to the car equipped with electric contacts such as
stipulated by ANSI A17.1-1965, Rule 204.4, American Safety
Code for Elevators. September 8, 1994.
