Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employee(s) in that open elevator door(s)
subjecting employee(s) to fall(s) which could result in their deaths:
a) Location: Sea Jet - A.P.A. Corporation, Warehouse Building, 3rd
Floor, Freight Elevator #1.
Among other methods, one feasible and acceptable abatement method to
correct this hazard is:
To equip each elevator door with a hoistway door interlock to prevent
operation of the driving machine by the normal operating device
unless all hoistway doors are in the closed position, as referenced
in American National Standard Safety Code for Elevators and
Escalators, ANSI/ASME A17.1 1984- Rule 111.4c - 1/7/88.
