Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to
crushing injuries due to an unsafe overhead hoist in use:
a) On or about 5/29/90, employees working in the north dock
area were exposed to an overhead coffin hoist being used; in
that, it was unsafe due to a deficiency in the braking
mechanism and hook that was not recognized because it was not
examined before use.
Among other methods, one feasible and acceptable abatement method is to
develop and maintain an adequate inspection procedure incorporating both
frequent and perioidic inspection guidelines for the coffin hoist.
