General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: On or about 1/28/88, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) Employees waiting for and walking by Building #22-2nd floor. Freight elevator hoistway were exposed to hazards of falling into shaftway and/or being struck by moving elevator car. One feasible an acceptable method among others to correct this hazard was to install and maintain hoistway interlock/mechanical lock system so that 2nd floor door could not be opened when car was not at landing, such as that stipulated by ANSI A17,1-1978 sections: 300-1; 111 rule 111.4e and 110.1.
