Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazard that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: a) At the main freight elevator, employees were exposed to the hazards of falls through elevator shaft when opening the elevator doors, there was no effective interlock to prevent employees from entering the elevator shaft when the elevator car is at another floor level. Employees are also exposed to being struck by the elevator when looking into the shaft to find the elevator. A feasible and useful method to correct the hazard among others, is set forth below. Provide an electrical interlock to prevent employees from entering the elevator shaft when the elevator is at another level and to prevent employees from opening the doors to locate the elevator. Abatement shall be in accordance with the American National Standard Institute (ANSI) A17.1-1987 Safety Code for Elevators and Escalators Rule III.I.
