Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that employees were exposed to:
On or about March 3,1986 in Bldg. K at Galimat 80 where a chemical
mixture was planned, the employer had not posted or otherwise
provided adequate written instructions or checklist to inform
employees of safe operating limits of temperatures to be maintained
in the Fluxing Chamber to control thermal decomposition, combustion
and fire propogation.
A feasible and acceptable abatement method is to post operating
instruction at Galimat Station and instruct each operator before each
run.

On or about March 3, 1986 in Bldg. K at Galimat 80 Fluxing Chamber
instruments.  Temperature meter was inoperable and not used to
control fluxing of plastic (PVP) mixture from thermal decomposition,
combustion and propogation of fire exposing employees to burns,
Hydrocloric Acid, Carbon Monoxide, Carbon Dioxide and and variety of
polymar fumes which may lead to "Polymar Fume Fever".
A feasible and acceptable abatement method is to include temperature
meter in routine maintenance safety check.
