Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to amputation and or crushing hazard:
a) 1348 Adams Avenue, Philadelphia, PA 19124 - employees use the 4 freight
elevators as a means of transportation from floor to floor.  This freight
elevator
has no interior doors or gates so when the elevator travels up and down the
employees are exposed to having the limbs of the their body caught between
the
elevator and the building structure.  Observed 03/02/94.
Note: One feasible means of abatement would be not to allow employees to
use
the freight elevator as a means of transportation.

used to
move rolls of fabric was missing a safety latch on the hook which is in
violation
of ANSI B30.16.- 1973 Section 16 - 1.1.2.4.  Observed 03/02/94.
