Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to:
a)  The workplace was not kept free from recognized hazards in that the
employer
required employees working as unit operators at the #9 stabilizer reboiler
to work under
extremely hazardous conditions without benefit of adequate illumination.
The entire
work area was flooded with approximately eight (8) inches of hot
condensate (measured
& 164 degrees farenheit several hours after the accident) that was
discharged from
process equipment.  An operators' work station was completely flooded with
the hot
condensate and was located immediately adjacent to a ditch (2 feet wide by
approximately
24 to 36 inches deep) that was completely submerged by the same hot
condensate.
Illumination in the work area was extremely poor due to a burned-out
light(s).  Steam
from the hot condensate further hindered visibility.  Failure to abate the
prevailing
hazardous conditions, including the inadequate illumination, resulted in a
unit operator
receiving second degree burns to both legs as a result of inadvertently
stepping into the
ditch full of hot condensate on 5/4/94.  The association between poor
illumination levels
and industrial accidents is clearly specified in the American National
Standard Practice
for Industrial Lighting (ANSI A11.1-1973).  Observed 6/10/94.
