Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death of serious physical harm to
employees in that employees
were exposed to falling loads and/or falling hoist parts:
a) Burn Shop, 5 Ton Column Mounted Jib Hoist - the wire ropes between the
block and the hoist motor were allowed to be used while in an unsafe
condition.
The number of broken wire in each strand of each lay were too numerous to
count. This condition was present along the entire length of the ropes.

b) Burn Shop, 5 Ton Column Mounted Jib Hoist - the company did not have an
inspection and maintenance program that included this hoist.  Observed
10/14/94.
c) Inspection Department - overhead cranes with a homemade unrated lifting
hooks were being used to lift objects weighing up to 1500 pounds.  Observed
10/14/94.
NOTE - A feasible method of abating the hazards in instances "a & b" would
be
to replace all damaged wire ropes and include this hoist in a regular
inspection
and maintenance program. To abate the hazard in item "c", have a qualified
individual determine the safe lifting capacity of the homemade lifting
hook.ng
