Section 5(a)(1) of the Occapational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which were
free from recognized hazards that were causing or likely to cause death
or serious physical harm to employees in that:
a) On 10/12/88, employees were exposed to the hazards of a breach of
containment of liquid anhydrous ammonia from the 15,000 lb. anhydrous
ammonia storage tank adjacent to the S/E wall of the Lucas Avenue
plant due to an over-pressurization caused by the failure of the
two safety-relief valves on the tank which could result in chemical
burns and/or asphyxiation by anhydrous ammonia.
One feasible and acceptable abatement method, among others, to correct
this hazard is to inspect and test the safety - relief valves at
intervals necessary to maintain the equipment in safe operating
condition.  The valves should be removed from service for testing at
the time of internal inspection of the anhydrous ammonia storage tank,
but at an interval not greater than 10 years, as stipulated in the
National Board Inspection Code, Section U-106 and 4-111.
