Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
S/C the hazard of contact with moving machine room parts.  One
feasible and acceptable method, among others to correct this hazard
is to prevent unauthorized employees into the machine room by
providing the doors with  spring type locks that meets the
requirements of ANSI/ASME A 17.3-1986, Part II-Sec. 2.2.2
a)   Location:  594 Broadway, Crosby Street Side- Elevator #1
& #2 Machihne Rooms.
The access doors were not provided with spring type llocks
which could be opened from the inside without a key on or
about March 26, 1991.

levels.  One feasible and accaptable method among others to correct
this hazard is to provide the elevator car with a top enclosure which
meet the requirements of ANSI/ASME A 17.3-1986, Part III Sec. 3.4.1(c)
a)   Location: 594 Broadway, Crosby Street Side
Elevator #2 was not provided with a top enclosure, on or
about March 26, 1991.

employer did not furnish employment and a place of employment which
were free from recognized hazards that were causing or likely to cause
death or serious physical harm to employees in that employees were
exposed to:
S/C the hazard of contact with pinch and shear points as a result
of an unintended operation of the elevators.  One feasible and
acceptable abatement method, among others to correct this hazard is
to stop using a rope operating device as required by ANSI/ASME
A 17.3-1986, Part III Sec. 3.10.1, and replace it with an "enclosed
electric type" operating device meeting the requirements of ASME.

Elevators #1 & #2 were operated by a rope manually operated
device prohibitted by the above mention code, on or about
March 26, 1991.
