The following citation was amended as follows: %% 1. Citation 02, "Repeat" was changed to read Citation 02, "Other" 2. Change wording Citation 2, Item 1 AVD (variable information) instance (b) was added. Executive Order 12196 Section 1-201(j): The head of the agency did not operate an occupational safety and health management information system which shall include the maintenance of such records as the Secretary may require: a) The log of occupational injuries and illnesses (OSHA-2014) or it's equivalent was not complete in the detail provided in the form and instructions contained therein: Deficiencies found in the 1991-1992 OSHA-200 logs of occupational injuries and illnesses were: 1. Tuberculosis skin test conversations were not recorded in the log. b) Entire establishment - The log of occupational injuries and illnesses (OSHA 2014), or it's equivalent was not complete in the detail provided in the form and instructions contained therein: Deficiencies found in the 1991-1992 OSHA-200 logs of occupational injuries and illnesses were: 1. Bloodborne pathogen exposures such as needlesticks were not described so as to indicate whether the needle was sterile or contaminated. 2. Lacerations were not described so as to indicate whether the cause was a medical instrument or other device. 3. Splashing of body fluids were not described so as to indicate whether the fluid was blood, saliva or some other potentially infectious materials. Both the amended citation and the attached original citation must remain posted for 3 working days. The employer has 15 days from the date of receipt of the amended citation to contest those portions of the original citation which have been amended. The contest period is not extended for the unamended portions of the original notification.
