Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likley to cause death or serious physical harm to employees in that employees were exposed to: a) Employees operating or riding passenger/ freight elevator at 55 Warren Street Street, New York, NY, rear, were exposed to falling into elevator shaft. The hoistway door electric interlock contacts were inoperable and in disrepair, while the elevator was still in use, indicating a defeated interlock system. ABATEMENT NOTE: A feasible and acceptable abatement among others to correct the hazard is to follow ASME/ANSI A17-3 1986, Section 2.7.1 (a) and equip hoistway doors with functional interlocks.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likley to cause death or serious physical harm to employees in that employees were exposed to: a) Employees operating or riding in passenger/ freight elevator at 55 Warren Street, NY, rear of premises, were exposed to falling into the elevator shaft or being caught between cab and shaftway sills or other portions of shaftway. The electric elevator cab did not have a cab gate. Observed 12/14/92. ABATEMENT NOTE: One feasible and acceptable abatement method among others to correct this hazard is to take the elevator out of service and install gate as called for in ASME/ANSI A17.3 1986 Section 3.4.2(a) ASME/ANSI 17.1 1989 section 111.5(b), 111.5(c)(1), 111.5(c)(2), 204.4(a) and (b).
