Section 5(a)(1) of the Occupational Safety and Health Act of 1990: The employer did not furnish employment and a place of employment which were free from recoginized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to possible burns and injuries caused by fragments due to possible leakage, rupture, or failure of dryer cans containing steam and condensate. The following conditions contributed to the existance of this hazard: a. Paper Machines 1 and 2: A program of perodic inspections, and documentation of inspection results of dryers/dryer cans was last implemen- ted in 1977. AABATEMENT NOTE: Among other methods, one feasible and accepted method is to implement an unfired pressure vessel program in accordance with TAPPT, 1988, TIS 0402-16, Paragraph E. Key elements of this five year program consist of the following: - A detailed internal and external visual inspection check conducted by experienced personel. - Magnetic particle testing and inspection of dryer heads conducted by trained and certified personnel. - Ultrasonic testing and inspection of at least 20of the head bolts for shank cracks conducted by trained and certified personnel. _ Ultrasonic thickness testing measurements conducted by trained and certified personnel on any parts of the shell that appear to be thinned by corrosion or wear, as revealed by internal visual inspection. ABATEMENT SCHEDULE Step 1 Develop a written plan and submit it to the Area Director. Step 2 Complete the Abatement Plan and evaluate its effectiveness. Note: Each inspector shall have education and experience equal to at least one of the following: 1. A degree in engineering plus 1 year of experience in the design, construction, repair, operation, or inspection of boilers or prussure vessels. 2. A two year certificate in engineering or techology from a technical college plus 2 years of experience in the design, construction, repair, operation, or inspection of boilers or pressure vessels. 3. The equivalent of a high school education plus 3 years of experience in the construction, repair, operation, or inspection of boilers or pressure vessels.
