Section 5(a)(1): of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to: CONFINED SPACE HAZARDS: AIR COOL CONDENSER STEAM HEADER WAS BEING ENTERED THROUGH A MANHOLE OPENING FOR THE PURPOSE OF PERFORMING WELDING OPERATIONS INSIDE THE HEADER WITHOUT THE IMPLEMENTATION OF A CONFINED SPACE ENTRY PROGRAM. ENTRY WAS MADE INTO HEADER PRIOR TO ATMOSPHERE TESTING, AND EMERGENCY REMOVAL PROCEDURES HAVING BEEN PUT INTO EFFECT. AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THIS HAZARD IS TO IMPLEMENT THE COMPANY'S EXISTING CONFINED SPACE ENTRY PROGRAM TO INCLUDE BUT NOT LIMITED TO REQUIRING EMPLOYEE TRAINING IN ATMOSPHERE TESTING, PERIODIC RE-TESTING, EMERGENCY REMOVAL AND FIRST AID.
